When we sent in our water license application (in person) we were handed a "Record of Self-Assessment Against Fisheries and Oceans Canada Project Submission Requirements" form. We were told EVERYONE who applied for a water license on Sakinaw was given one to complete, or it was mailed out if they mailed in their application.
